Knights and Monarchs players require the following forms prior to the start of the season:

Permission to Travel Form - please attach copy of parent’s drivers license (who signed the form)

A copy of players passport

Majestics and Aces do not require additional forms beyond their initial registration.

Completed forms maybe returned to the Jeffries office or by emailing info@medicinehatbaseball.com
